Observations and Near Misses

Safety Observations is where safe and at-risk conditions and near misses get reported by whoever sees them, from a login or from a QR code with no login at all. It is your leading indicator: the hazards found before anyone was hurt.
1.Report an observation from the app
- Open Observations. The Report an Observation form is on the page.
- Choose Safe or At-Risk, then a category (PPE, Housekeeping, Fall Protection, Electrical, Equipment / Tools, Ergonomics, Environmental or Behavior) and a severity (low, medium, high or critical).
- Pick the job site and describe what you saw.
- Add a photo if it helps. A photo taken on a phone records where it was taken.
- Press Submit Observation. With no signal it is saved on the device and uploads when you are back online.
2.Post the QR code for crews
- Press Field QR Code and then Print, or Copy link to share it another way.
- It is one code for the whole company. A worker scanning it fills in the same form on their phone, with the site, their name and a photo all optional.
- We list a report with no name as Anonymous.
3.Turn an at-risk observation into a corrective action
- Find the observation in the list. On an open at-risk card, press + Corrective Action.
- Give it an owner, a due date and a priority, and create it.
- We show on the card how many corrective actions hang off it, and the action is tracked in the Corrective Actions module until it is closed.
4.Work the list
- Click a card to open it and edit the details.
- Filter by type, job site, severity, status or date, or search the description. Press Clear Filters to reset.
- Press Close when an observation has been dealt with, and Reopen if it comes back.
Common questions
- Can workers see what other people reported?
- No. The QR page only submits. Reading the list needs a login.
- Is there a size limit on photos?
- A photo submitted from the QR page must be under 3 MB. Phones usually produce photos within that; if one is rejected, take it again at a lower resolution.
- Where do observations show up elsewhere?
- This month's count is on the dashboard, recent ones are listed there, and they feed the leading indicators in Statistics and the weekly summary.
