Incidents and the OSHA 300 Log

Incident Reports is where injuries, illnesses, near misses, property damage and other events get recorded, investigated and closed. What you enter here feeds the OSHA 300 log, the 300A annual summary and the 301 form, so nothing is retyped in January.
1.Report an incident
- Open Incident Reports and press + Report Incident.
- Choose the type: Property Damage, Injury, Near Miss, Illness or Other. Set the severity (1 Minor, 2 Serious, 3 Severe), the date, the job site and the specific location.
- Pick the person involved and who reported it from the roster.
- Describe what happened. If the AI tools are on, Draft with AI lets you describe or dictate it in your own words and fills in the form for you to review and edit.
- Under Investigation, record the immediate actions taken and the root cause when you know it.
- Attach photos or documents. A photo taken on a phone records where it was taken, and the record shows a map link.
- Add corrective actions on the form. Each one is tracked in the Corrective Actions module with its own owner, due date and status.
- Press Save. With no signal, the report is saved on the device and uploads when you are back online.
2.Decide whether an injury or illness is recordable
- With the type set to Injury or Illness, the OSHA recordable section appears on the form.
- If you are not sure, press "Not Sure? Walk It Through". It asks the questions in the order 29 CFR 1904 asks them and cites the section behind each answer.
- The walkthrough advises and pre-fills. The decision is yours: it never unticks anything you have set, and you can change any answer.
- If the case is recordable, tick OSHA recordable and choose the classification: Death (column G), Days away from work (H), Job transfer or restriction (I), or Other recordable (J). Enter days away (K) and days restricted (L), and the case category (M).
- Tick Privacy case when 29 CFR 1904.29(b)(7) applies. The worker's name is then withheld from the printed log.
- Save. We add the case to the OSHA 300 log for that year.
3.Investigate, then close
- Open the incident from the list and fill in the Investigation section as you learn more.
- Add a corrective action for each thing that has to change, with an owner and a due date.
- When the investigation is done, set the status to Closed and save.
- A closed incident is read-only. Only an admin can reopen it, using Reopen to Edit, and we write every change to the Activity Log.
4.Keep the 300 log and post the 300A
- Open OSHA 300 Log in the sidebar and pick the year at the top right.
- Fill in the establishment information once: name, street, city, state, ZIP, industry description, NAICS code, annual average number of employees and total hours worked for the year. The 300A needs these.
- Review the case log. Each recordable case appears with its classification and days; privacy cases show a case number instead of a name.
- Press Print Forms 300 / 300A. The printout follows the layout of OSHA's Form 300 and Form 300A, ready to certify and post from February 1 to April 30.
- Press Download CSV to take the year's log into a spreadsheet.
5.Produce a report for a carrier or a client
- Open the incident and press Print Report or Download PDF.
- For a recordable case, press OSHA 301 to open the incident report in that form's layout.
- A recordable, lost-time or fatal case can only be edited or exported by an admin. A user can still read it.
Common questions
- Does Migna decide whether a case is recordable?
- No. The walkthrough advises and pre-fills the form, and the printed forms reflect whatever you decided. You make the recordability decision and can change any answer at any time.
- Who can see incidents?
- Everyone with a login sees every incident. The job site on a login only affects which alerts that person receives, not which records they can open.
- What is a privacy case?
- Under 29 CFR 1904.29(b)(7) you must withhold the name on the 300 log for an injury to an intimate body part or the reproductive system, a sexual assault, a mental illness, HIV, hepatitis or tuberculosis, a needlestick or sharps injury contaminated with another person's blood or body fluid, or another illness the worker asks you to keep private. Tick Privacy case and the printed log shows "Privacy Case" in place of the name.
- Do near misses go on the 300 log?
- No. Only cases you have marked recordable appear on the log. Near misses stay in Incident Reports and count in your statistics.
- Can I export the list?
- An admin can press Export CSV on the incident list; the file follows the filters you have set. The full account export in Company Settings includes every incident and attachment.
